India’s Record Heat Waves Reveal Alarming Undercount of Fatalities, Highlighting Challenges in Crisis Management and Prevention

BENGALURU, India — As the region breathes a brief sigh of relief from severe heatwaves, with temperatures this year frequently soaring above 50 degrees Celsius (122 Fahrenheit), the true toll of heat-related casualties in India has become a critical concern. Public health experts estimate fatalities may reach into the thousands, revealing a significant gap in the official figures which only account for a fraction of the deaths.

The disparity between reported numbers and the likely actual count of heatwave fatalities underscores an emerging crisis in public health monitoring and disaster preparedness. Estimates suggest that recorded temperatures have hit unprecedented highs across the country, particularly affecting vast areas in the north where the official count acknowledges only 110 heat-related deaths.

Dr. Srinath Reddy of the Public Health Foundation of India stresses the severe underreporting largely stems from systemic issues in how deaths are recorded and classified. Heat often plays a significant but indirect role in mortalities, particularly among vulnerable populations such as the elderly or chronically ill, who may succumb to heat even when indoors. However, these factors are frequently overlooked or unrecorded in death certificates, skewing the official data.

The challenges are compounded by health infrastructure limitations. In many public hospitals, where resources and staff are already stretched thin, the immediate cause of death is prioritized in records, and environmental factors like heat are seldom included. This not only misrepresents the actual impact of heatwaves but also hampers effective policy-making to combat and mitigate future crises.

Furthermore, despite national guidelines established to strengthen the recording of heat-related fatalities, adherence remains inconsistent. This inconsistency was illustrated when Dileep Mavalankar, former head of the Indian Institute of Public Health in Gandhinagar, noted the disparity between reported heat stroke cases and deaths. Officially, there are about 40,000 cases of heat stroke reported annually, yet deaths recorded are suspiciously low at around 0.3% of these cases, a fraction of the expected 20 to 30% fatality rate typically associated with heat strokes.

Recognizing the urgent need for accurate data, efforts have been made to improve disaster preparedness through localized initiatives. In fact, the city of Ahmedabad pioneered India’s first-ever heat action plan in 2013, following a particularly deadly heatwave. The multifaceted strategy included enhancing shade coverage in public areas, transforming public buildings into cool shelters, and ensuring hospitals were equipped for heatwave crises. Following the implementation of these measures, fatalities during heatwaves dropped by as much as 40%, showcasing the potential benefits of well-planned interventions.

However, implementing such comprehensive strategies nationwide is hindered by the lack of reliable data. Without accurate figures, many areas remain underprepared, unable to implement measures proven effective in places like Ahmedabad.

Data discrepancies also extend to national level reporting by various governmental agencies. The disparities in data collected by different authorities highlight an urgent need for a unified approach to data collection and sharing. This would enhance understanding of the impacts of extreme heat and aid in developing targeted interventions to protect those most vulnerable.

Experts argue that the path forward must involve a systemic overhaul of how environmental factors are integrated into public health data. By acknowledging and addressing these gaps, India can better prepare for and respond to the increasing threats posed by extreme weather conditions, safeguarding the health and well-being of its populations for future generations.
