Kerala Tops National Chart for Communicable Disease Fatalities, Leptospirosis Major Culprit

Kochi, India – Kerala has recorded the highest number of deaths from communicable diseases among Indian states, a recent report from the Union government reveals. With 332 fatalities reported from January 2021 to December 3, 2024, this southern state surpasses even the more populous Maharashtra, which counted 331 deaths in the same timeframe. The concerning figures emerge despite Kerala’s lower population of approximately 33.4 million, compared to Maharashtra’s 100 million.

Leptospirosis, a bacterial infection typically transmitted through water contaminated by rodent urine, was identified as the leading cause of death, responsible for 103 of the total reported fatalities in Kerala, according to Union government data. However, recent state government statistics note an alarming increase, with 220 deaths attributed to leptospirosis in 2024 alone. These numbers underscore the varying impacts of communicable diseases across different regions and populations.

A senior public health official attributed the rise in disease cases in Kerala to several environmental and social factors unique to the region. “Kerala’s dense vegetation and abundant water bodies create an environment conducive to the spread of diseases through host agents,” the official explained, emphasizing that the state’s high population density further complicates disease control efforts. In contrast, many communities in northern India are more spread out, which can help contain the spread of infections to localized areas.

The official further noted a decline in disease resistance linked to increasing comorbidities driven by lifestyle changes. “The shift in living standards has contributed to a reduction in innate immunity among the population,” they stated, highlighting challenges in maintaining public health in the face of evolving environmental and social conditions.

Historically, Kerala did not frequently report cases of dengue fever, chikungunya, and H1N1. However, these infections have now become endemic to the state, with climatic conditions favoring their persistence and expansion. This increase in disease prevalence reflects the vulnerability of the population, including delayed treatment and inadequate preventive measures such as the underutilization of distributed prophylactic doxycycline tablets by those at risk.

Dr. Rajeev Jayadevan, convener of the research cell at the Indian Medical Association in Kerala, pointed to the state’s robust healthcare system as a factor in the high number of reported fatalities. “Kerala’s healthcare infrastructure, which extends to the grassroots level, facilitates thorough detection and reporting of healthcare cases,” Dr. Jayadevan explained. He noted that while reported cases reflect only a fraction of the actual disease burden, Kerala’s efficiency in case reporting is relatively high compared to other states.

This effective health system, combined with high literacy rates, broad healthcare access, and significant levels of women’s empowerment and self-sufficiency in Kerala, contributes to successful health outcome reporting and management.
