The men were left in a running vehicle unattended by staff at the facility, an incident that has prompted state officials and advocacy groups to call for a thorough investigation.
The Utah Department of Health and Human Services has initiated emergency actions against Safe and Sound Services, LLC, in the wake of this incident. Multiple state agencies are now conducting their own inquiries as concerns about client safety rise. Nate Crippes, a representative from the Disability Law Center, expressed sorrow for the men’s families, emphasizing the preventable nature of the tragedy. “It’s a shocking situation, and no one should have to face such a loss,” he remarked.
According to a probable cause statement, Isaiah Pulu, a 25-year-old staff member at the facility, left 25-year-old Colton Moser, 22-year-old Mosa’ati Moa, and 39-year-old Tim Jones in a running vehicle while he took a break at his apartment for several hours. When Pulu returned, he discovered the men unresponsive. This tragic scenario underscores critical lapses in oversight and care protocols.
In response to the incident, the Health Department has imposed stringent conditions on the facility’s operations. Safe and Sound Services is prohibited from accepting new clients for the next 30 days, during which time the state will examine their practices more closely. The organization is required to comply with inspections and must immediately retrain all staff on safe transportation and supervision methods.
The state’s notice of action highlights the urgency of rectifying procedures within the facility. As investigations continue, the department may pursue further action based on their findings. Meanwhile, Safe and Sound Services has taken down its website, which currently displays a message indicating it is “under maintenance.”
